RELEASE AND WAIVER OF LIABILITY AGREEMENT
Waiver: I hereby release Elite Racing, Inc., its affiliated corporations and charities, the host city(ies), county and state, USATF, any and all municipal agencies whose property and/or personnel are used or in any way assist, all sponsoring or co-sponsoring companies or individuals related to the Event, together with their officers, directors, shareholders, successors and assigns, from responsibility for any injuries or damages I may suffer in connection with my participation as a volunteer in the Event. I agree to the use of my name and photograph in broadcasts, newspapers, brochures and other media without compensation. As a participating volunteer, I certify all information provided in this form is true and complete.
VOLUNTEERS MUST BE 14 YEARS OLD BY 4/25/08. RETURN BY 4/11/08 TO ENSURE VOLUNTEER POSITION.
